Nepal's Three Passes trek - how difficult is it really?

I'm headed off backpacking through Asia soon and will be passing through Nepal. I've heard really good things about the Three Passes trek - it's exactly what I'm looking for. However, it's got a reputation for being quite challenging compared to some of the other treks in the area. I'm fit and have some hiking experience but I've never hiked at serious altitude before (the highest I've gone is 1500m, compared to ~5500m on the trek), or on a trek longer than 3 days. The distance/elevation maps I've seen online would be about the edge of my fitness level at sea level. For anyone who's been, is it achievable for a mountains novice or should I be aiming for something more reasonable like Annapurna/EBC instead? I'm not time limited so I have plenty of room for acclimatisation or taking it slow.

Also, if anyone has some recommendations for high-altitude hiking insurance for Australians, it would be much appreciated! I've gone with World Nomads for most of the trip but I've heard they're not good for high altitude trekking due to their requirement to call them first before getting evacuated.
