EU citizen here who's been to Iran within the last 5 years. I want to visit a professional conference in the US. What would be the right procedure?

Been reading on the websites of the US Embassies in the UK but am still uncertain about the actual cost, duration, and the visa I need.

It looks like I need to apply for the visa waver programme, be denied and then apply for a visa using the form and then making an appointment?

Costs?

Duration?

Can i extend a business trip with a week long vacation or is that a no?

Which visa do I need? It looks like this would be a business visa, right?

Would the US embassy/border control have a problem with an old work permit for Qatar, and stamps for pretty much all Arab Gulf countries in my passport? I will not be able to get a replacement passport unless I travelled to Israel and showed an existing flight ticket.
